History: 74 yo male with acute abdominal pain
Solution: Cecal volvulus is an emergency that requires emergent surgical intervention because of the likelihood of vascular compromise, ischemia and perforation. Just like other volvuli, it is a closed loop obstruction that can progress quickly. The torsion usually occurs within the ascending colon just above the iliocecal valve and there is often an associated partial malrotation. The imaging findings are pretty obvious in this case with the cecum being dilated, located in the upper mid abdomen and having evidence of ischemia on the cross-sectional imaging.
