Duodenal Adenocarcinoma With Krukenberg Tumor

History: 78 yo female with early satiety
Solution: Duodenal carcinoma is similar in histology to other GI adenocarcinomas. Most common presenting symptom is abdominal pain due to obstructive symptoms. Is also associated with N/V, weight loss, anemia, Upper GI bleeding and can cause obstructive jaundice. Spread is usually by direct extension with hematogenous and lymphatic metastases also possible. Spread to the ovaries is seen in multiple GI cancers and the acronym is Krukenberg tumors. These are carcinoma of the ovary, usually metastatic from gastrointestinal cancer, marked by areas of mucoid degeneration and by the presence of signet-ring–like cells. With metastatic disease like this, the primary treatment option is palliation with radiation, chemotherapy, and stenting if necessary.
