Peliosis Hepatis

History: 35 yo female with fever, sweats, and abdominal pain.
Solution: Peliosis hepatis

This is a rare benign disease. The findings are related to sinusoidal dilatation and presence of multiple blood filled lacunar spaces within the liver. In this case, there was a solitary lesion. The patient's symptoms were likely not related to this lesion, but were related to here underlying HIV/AIDS, which can predispose to bacillary peliosis and angiomatosis.

These are most commonly asymptomatic. They can be associated with anabolic steroid use, corticosteroids, tamoxifen, oral contraceptives, or can be seen after renal/cardiac transplant, and can be congenital.

On imaging, they usually show centripetal, progressive enhancement, which is often continuous, helping to differentiate these masses from hemangiomas. They are often more homogeneous than adenomas, although these two entities cannot always be differentiated on imaging alone. These lesions also show strong enhancement on delayed images, which can be helpful if delayed imaging is available.
