History: 35 yo female with pain 2 weeks after IUD placement
Solution: Finding on this exam is relatively obvious in that the IUD is not positioned within the endometrial canal, but appears to be outside the uterus and within the peritoneal cavity. There is some associated free fluid, but not a large amount of hemorrhage on these US images. The patient subsequently underwent a CT to confirm the findings.
IUD's are an effective and relatively safe form of birth control, but can be associated with irregular bleeding, pelvic pain, perforation, infection, expulsion, and pregnancy. Remember that there are variable configurations of IUD's and 3D imaging can be helpful for confirming IUD positioning.
