Intra-pancreatic Splenule

History: 60 yo male for follow up of abnormality identified on trauma CT 2 years previously.
Solution: An intra-pancreatic splenule or accessory spleen is a rare finding, but can often be disgnosed if one thinks about it. In this case, we have a pancreatic tail mass that is stable for two years and is enhancing indentically to the adjacent spleen. Although one has to consider other possibilities, this should raise the possibility of an accessory spleen in this location. The diagnosis can be confirmed with nuclear scintigraphy (technitium sulfur colloid) if needed and biopsy should not be necessary.
