Case: 139

Clostridium Difficile Colitis

Show

History: 68 yo male with history of diverticulitis and worsening abdominal pain.
Solution: The art of reading plain films is becoming a dying art because it is all too easy to just recommend cross sectional imaging. However, these plain films are a terrific example of "thumbprinting" of the colon in the setting of colitis. This is non-specific in that many processes can cause colonic wall edema and thumbprinting, but it is specific/pretty suggestive of colitis. In this case, the patient had been on antibiotics for the diverticulitis and ended up with C. diff colitis. On the CT

was a pancolitis, which makes ischemic and inflammatory causes less likely.
