Small Bowel Volvulus After Total Colectomy

History: 35 yo male with severe abdominal pain after a prophylactic colectomy
Solution: The findings on these images include poor enhancement of a segmental portion of the small bowel that is mildly dilated. In addition, you can see that there is swirling in the mesentery and if you look closely you will note that as the mesentery spins on itself, the superior mesenteric vein is occluded. The SMA remains patent. This is a closed loop obstruction that is secondary to the surgery and represents a surgical emergency to prevent bowel infarction and perforation.
