History: 17 yo male with right testicular pain x 18 months.
Solution: In male patients with Congenital Adrenal Hyperplasia (CAH), adrenal rests are often present in the testicles. This condition is usually due to 21-hydroxylase deficiency and is associated with impairment of cortisol and aldosterone production, resulting in an increase in the secretion of ACTH by the pituitary. This leads to hyperplasia of the adrenal cortex and production of high amounts of adrenal androgens. Testicular adrenal rest tumors can be the first manifestation of CAH, but more commonly, the diagnosis is known prior to this diagnosis. Clinically, female patients with this disease have virilization of the external genitalia. Both genders have rapid linear growth and bone age advancement, which leads to short stature if not treated. Testicular adrenal rest tumors can be associated with infertility and sometimes are surgically removed.
