History: 31 yo pregnant female with R sided abdominal pain.
Solution: In pregnancy, when a patient is thought to have possible appendicitis, most people would start with an US (done and negative in this case) and progress to MRI as needed. The diagnosis is made based upon the same imaging findings, i.e. enlarged appendix with periappendiceal inflammation. Note that gadolinium cannot be administered, so these images are all non-contrast. There are numerous protocols for performing MRI imaging for appendicitis during pregnancy. The most useful sequence is often the SSFSE, which can be obtained in multiple planes.
