History: 48 yo female with abdominal pain and fevers.
Solution: This is a nice example of pyosalpinx. There is a tubular, fluid filled dilated structure in the right adnexa with adjacent fluid and inflammatory stranding. The appendix is seen higher and appears normal. Although this does not appear to have developed into a tuboovarian abscess, this patient is certainly at risk. Although antibiotics may be effective for treatment, surgery is often needed.
