Pyeloureteritis Cystica

History: 58 yo female with hematuria
Solution: Pyeloureteritis cystica

The retrograde pyelogram identifies multiple tiny radiolucent filling defects involving both ureters, as well as the left renal pelvis. These are characteristic findings. These small cysts most frequently involve the proximal 1/3 of the ureter, but can also invole the renal pelvis and bladder. The cysts contain translucent or yellowish fluid, and may be a response to chronic urinary tract infections, although the exact underlying etiology is unclear. They are most frequently asymptomatic, but are associated with urinary tract infections. They are not associated with obstruction of the ureter or malignant degeneration. Differential considerations should include: air bubbles, multifocal TCC, and metastatic melanoma.
