Utricle Cyst

History: 28 yo male with low back pain.
Solution: Utricle cyst

There are multiple different kinds of prostatic cysts, but the majority are either Mullerian duct cysts, or utricle cysts. These are both midline lesions, and differentiation between the two is often based upon the size and location of the cyst. Mullerian duct cysts are usually larger and extend above the base of the prostate, often in a paramedian location. Utricle cysts on the other hand are usually 8-10 mm in size and do not extend either laterally, or above the prostate. Utricle cysts are most commonly an incidental finding, but Mullerian duct cysts can cause symptoms. Although asymptomatic, utricle cysts may be associated with other urogenital anomalies such as hypospadias, undescended testis and renal abnormalities including dysplasia. If aspirated, mullerian duct cysts will not contain spermatozoa, but a utricle duct cyst may. Both can become infected, hemorrhage or undergo malignant tranformation.
