Hepatocellular Carcinoma

History: 63 yo male with weight loss and early satiety.
Solution: Hepatocellular carcinoma The most common primary malignant tumor usually arising in a cirrhotic liver. Mass is usually large and heterogeneous, with hypervascularity seen on arterial phase imaging. Has three primary growth patterns; solitary, nodular and multifocal, and diffuse. On imaging, fat, necrosis, and calcification can all be seen with HCC. There is usually a capsule associated with the mass, and it almost always shows hypervascularity on arterial phase imaging. MR findings include: hypointensity on T1, hyperintensity on T2, enhancement on arterial phase with washout on portal venous phase. This lesion shows heterogeneous appearance and does have fatty attenuation. There is a capsule and there is brisk arterial phase enhancement. This is a typical appearance for HCC. Prognosis with a lesion this large is dismal.
