History: 26 yo female with headaches.
Solution: Polymicrogyria is a migrational disorder, in this case affecting the right frontal lobe. Polymicrogyria is among the more common of the migrational disorders. Bifrontal and bilateral perisylvian patterns are recognized, as well as isolated sporadic cases, which is most likely in this case. Varying clinical manifestations are seen corresponding to the region of brain affected, but general associations include seizures and developmental delay. The headaches reported in this case are either unrelated or spurious more than likely. As imaging has improved, it is recognized that subtle forms of migrational disorder or heterotopia are more common than originally thought. Some recent studies have suggested that serial imaging, especially in the pediatric epilepsy population, is warranted in seizure patients to search for subtle underlying structural causes of epilepsy.
